Required Forms:

All forms can be found in the Registration Portal. Please remember to fill out and upload the following documents:

  • Personal Profile (to be completed by the participant)
  • Passport Copy (must not expire before: August 26, 2018 )
  • Health Insurance Card (front and back)
  • Health History (to be completed by participant's parent or guardian)
  • Physician's Exam & Immunization Records (to be completed by participant's physician)
  • Photo

    Two Recommendations, completed by: 
            1. The Synagogue Rabbi or Education Director AND 
            2. A Middle School Principal or Teacher

Financial Aid

The 2018 Financial Aid Application process will begin in the fall of 2017. Families must already be registered for Ta'am Yisrael to apply for financial aid. Applicants will be notified of awards by late-December, following the meeting of the Scholarship Committee. If you have any issues with the application, please call the FACTS help line at 866-441-4637, 7AM-9PM Monday-Friday and Saturday 8AM-2PM.
